360 Booth Participation Waiver & Media Consent

Welcome to our 360 Booth experience! Before participating, please read the following carefully.
By completing this form, you acknowledge and accept all terms listed below.

Participation & Safety Acknowledgment

I understand that the 360 Booth involves standing on a rotating platform while being filmed by an automated camera arm.
I agree to:

• Follow all safety instructions provided by booth staff.

• Keep clothing, hair, and belongings secured to avoid obstruction.

• Avoid jumping, pushing, or stepping off the platform while it’s in motion.

• Participate voluntarily and at my own risk.

I confirm that I do not have any health conditions (e.g. injury, dizziness, pregnancy, or balance issues) that would make it unsafe to participate.

Liability Waiver

I hereby release and hold harmless SpinShot Events, its owners, staff, and affiliates from any and all liability, claims, demands, or damages arising out of my participation in the 360 Booth, except in cases of proven negligence.

I understand that SpinShot Events is not responsible for any loss, damage, or injury that may occur during or after use of the booth.

Media & Marketing Consent

I understand that photos and videos may be taken during my participation and may be used for marketing or promotional purposes, including:

• Social media (Instagram, Facebook, TikTok, etc.)

• Websites and digital content

• Print or online advertising materials
